Mayra Cabrera, a 30-year-old nurse from Swindon, England, suffered a heart attack and died within three hours of giving birth to a healthy son, after she was mistakenly given a fatal dose of an epidural anaesthetic. The same fatal mistake (injecting the dose into the arm instead of the spine) happened to two previous women in British hospitals.
